Clinicians play a critical role in a dementia friendly community from identification of cognitive impairment and assessment to management of dementia. This survey can be completed by anyone in the community, particularly someone with contacts in the healthcare sector. Instructions: After completing this survey, develop a list of clinics we could contact to take this survey. Determine the appropriate person at the clinic to answer these questions e.g., physicians, physician assistants, nurse practitioners, nurses, nurse aides, social workers, care coordinators, etc. Send the copy of this survey or send your contact's information to Ingalsk@ccf.org.
