Community-Based Services and Support: Dementia Friendly Clark County Needs Assessment

Services and supports for family caregivers of people with dementia are needed in communities striving to become dementia friendly. They include home and community-based support organizations, social service organizations, community outreach and wellness organizations, organizations that provide counseling, support groups, caregiver coaching, care consultation, respite, education and training on dementia care needs.  Please complete this survey.

Dementia Friendly Clark County Needs Assessment: Community-based Services and Supports

Services and supports for family caregivers of people with dementia are needed in communities striving to become dementia friendly. They include counseling, support groups, caregiver coaching, care consultation, respite, and education and training on dementia care needs. This survey can be completed by anyone in the community, particularly someone who has had experience with caregiver support services. Instructions: Develop a list of agencies/organizations that provide services or support to caregivers such as adult day services, respite care, support groups or counseling. Prioritize and determine with organization we should contact.
